Passwords from CSV file not imported

I have not been successful in getting the passwords to be imported from my CSV file. I have read the documentation, and formatted my CSV file as described. As a separate test, I imported the test.csv file provided with the documentation with the same result. All 7 of the users have been imported from test.csv, and status has been set to approved, but get password error when attempting to log in.
