The password would be defined at level of page (in WordPress you can protect the access to the page’s content with a password), and to insert the shortcode into the content of this page.

You can limit the access to the forms only to registered users, even to specific roles, using the “Users Permissions” add-on. More information visiting the following post of the plugin’s blog:
