[Plugin: Participation Database] Multi Form Action not working

I have had a huge issue with permalinks, but as i think i have sorted them out, it seems to have stopped the action within the signup shortcode. I have the slug, but when the button is pressed to move to another page, it just goes to the home page. And within console it just says, Navigate to https:// home page, and not the correct form page.

Any ideas how i can see why its doing it ?
