Parent/Conditional Fields are Not Working

I am trying to have fields appear based on selections of other fields. I got it to work once. I have a Drop-down field for Request Type, if the user selects Computer, a field for Computer Number Shows. I want another field for Room Number of the person selects Room for Request Type. I cannot get it to select from Request Type in Parent. I can’t recall how I got it to work the first time.

Is there a certain process to have conditional field s work? I know it does work, I just can replicate it again.
