Page says "There is no input."

Hi There,
I have installed the plugin and created a new Form. When I put the short code on a post, the Form is shown correctly with all the fields.
But whenever I put the code on a page, no form is shown except the text “There is no input.”See the Page: https://www.languageclinicbd.com/data-entry
