Page not found error

Hello,

I have tried to using the plugin but when i try to upload image and submit the form it’s showing ‘Page not found’ error. Have updated permalinks but still the issue remains.

We have added the form as a popup using popup builder shortcode widget here is the website URL https://zelocare.com/zelocarewp/

Please visit the home page and move to bottom of the page and click on image that saying ‘Add your photo’ it’s opening popup.

Also i have checked with page https://zelocare.com/zelocarewp/post-image/ but here also nothing happening.

Please respond ASAP
