Our installs does not refresh permalinks, upon adding terms to a taxonomy -child

As described, our installs does not refresh the permalink struckture upon adding new child terms to existing terms in a custom taxonomy, which results in 404’s when a link to this new child taxonomy is clicked.
How can that be?
