Opting back into email reminder

First thing, this is a great plugin. Okay, the problem I have is a user accidentally opted out of receiving email reminders. He wants to get the emails again, but I can’t find how to add him back in. Earlier versions had the Email Reminder checkbox on the entry form, but that isn’t there with the latest version. How can I (admin) or the user opt back in for email reminders?
