optinform issue

  • Hi
    I added optin forms to use with my mail chimp and WP site.
    All seemed great but for some reason the form appears twice at the bottom of my page – once correctly at the bottom of the page and the other in a compressed form into the left side of the page?
    I am not sure how to fix this and any assistance would be greatly appreciated.
    I can’t include a screen grab but you can see it here at the bottom of the page.
