Optin page is on all pages!

    Hello you all good guys!
    I have placed an optin page in my front page of my website, https://www.weightlost4teens.com and it is showing on all of my pages. I placed the codes at the very bottom of the header.php in case this info is helpful. I dont want the optin to show on all pages, just the front page.
    I also would like the optin to show on the right hand sidebar rather below the header because having it here, it pushes all the post further below.
    Can anyone help me with this?

  • First, remove the code from header.php.
    Now insert the code into the sidebar widget. (Under widget option, Drag n drop the Text widget to sidebar and place the code there..)
