Optin Form differences between Setting and Webite

Hello Everybody!
I need to know why in my website the Optin form has differences from the setting in the admin WP page and the website itself.

This is how I can see the Optin Form in the setting menu:

And this is how you can see in the website:

PS: it worked until few days ago…and I didn’t change anything! O_O
