OpenID assertion failure when adding identity URLs

I’ve installed and activated the WP-OpenID plug-in. I’m having trouble adding identity URLs to an existing user account. When I attempt to add a URL, I get the following error message:

Error: OpenID assertion failed: Nonce already used or out of range
