Open automatically a ticket

Hi!
On my site I have to create a questionnaire for non-subscribed users.
This questionnaire serves to relate the user and an operator “x” of the site.
When the user clicks on “send the questionnaire”, I would like a ticket to open automatically. This ticket must be visible to the user who answered the questionnaire and to the operator “x” who has to answer this user.
Is that possible?
