Only receiving partial responses

Hi all,

I have the latest version of Contact Form 7 loaded with the latest version of WP. In this case, I am using CF7 for an assessment that has 15 forms to be filled in. Here is the test email I received…

As you can see, I should have received a lot more information on these tests but just the info from one box (number 3). Here is the post, https://devingalaudet.com/the-assessment/ . Do you have a suggestion to what I am doing wrong, or can there only be one form on each page? FYI, the bottom form was created by JotForms and is working, but I would much rather use CF7.

Any help is much appreciated.
