Only box and submit button displaying

  • I’m helping a client with a form (v 1.6.33), which we had no problem setting up on her portfolio site. She wanted the same form on her blog, so we installed a newer version (1.6.40) of the Form Manager and set up a new form. Inserting the shortcode into a page is resulting in just a box with the submit button displaying, however. I can’t figure out why none of the form elements are displaying. Anyone else have this problem/know how to fix it?
