Notification Not Always Being Sent

I’ve been using Custom Contact Forms blindly for some time now. I was getting various notifications. A recent problem caused me to log back into the Admin console and check some settings and there I noticed that I had not received notifications for several submissions. Any ideas why some would be sent and not others?
