    I hope you’re doing well today!

    Thank you for sharing the form export. I’ve tested it on my lab site, and could not replicate similar issues with the notification emails.

    Could you please install and activate WP Mail Logging plugin, and check new entries in WP Mail Logging >> Email Log section after submitting the form.

    In case there’s an error during the form submission, you should see it in the “Error” column. You can then check it further with the email hosting support.

    Please also try enabling WordPress debugging by adding the necessary define statements in your wp-config.php file as explained here:

    If you get any messages in /wp-content/debug.log file after submitting the form with attachments, please share this log file with us, as it may contain related debug info.
