Nothing happens on submit

I am trying to create a personality type quiz where each answer gets a score and depending on the total there are 5 different result pages.

At the moment nothing happens when I submit. Also the contact form does not appear even though it is set to appear at the beginning?

I have read through all the documentation and watched the tutorials but just can’t figure it out. I would be super grateful for any help.

I have the reCAPTCHA addon.

I don’t see anywhere to add images – do I need to have url’s for the images?

As per forum guidelines

For support of commercial themes or plugins, go to the official support channel.
In order to be good stewards of the WordPress community, and encourage innovation and progress, we feel it’s important to direct people to those official locations. Doing this will provide the developer with the income they need to make WordPress awesome.
Forum volunteers are also not given access to commercial products, so they would not know why a commercial theme or plugin is not working properly.

Ultimately, the vendors are responsible for supporting their commercial product.
If you are a vendor and observe someone asking questions about your paid plugin or theme, please direct them towards your own support resources.

Unfortunately I am not a pro customer, I am on a very limited budget and need to know the product works before I can commit to any expenses. Please can you help me resolve this issue?

I request you to please disable that QSM reCAPTCHA addon or configure it properly and then clear the browser cache and history and then check.

Please excuse the delay. Thank you very much. It seems the captcha was the reason, I changed the captcha version and it seems to have done the trick. I still do not see the contact form but the quiz does work and the results page does display.
