Nothing Happening When Registraion is Submitted

I have registration all set up and when I go in to make a registration, once the submit button is clicked, the page refreshes (with a blank query after the URL – e.g. “/registration/?”) and all guest info is lost and no registration is logged.
Have had good success with this plugin in the past but it is completely non-functional at this point… Any one else have thoughts / fix?
