As i said – the only plugin now activated is SGR Nextpage Titles – nothing else is active

I see but this doesn’t mean that it is the cause of not working. If you rally wanna test if SGR NextPage Titles is working for you, make a clean WordPress 3.5 installation and install only it. As I told you since 0.65 it automatically reflush rules on activation and on deactivation, but not all plugins reflush rules on deactivation, trust me! maybe some other plugin missed that reflush and now messed with permalinks. Instead of making a totally clean installation you could first try again deactivating “ALL” plugins and then reactivating only SGR NP Titles, this should give you a pretty clean permalink structure because on SGR NextPage plugin activation there will be a reflush rules.

I’ve done the latter already – I deactivated ALL plugins – then reactivated SGR, that means, from what you said, the rewrite rules will be flushed – it still does not work – I switched to a vanilla theme away from the main theme – still does not work

I give up – I just noticed another bug in it – when showing the content in the journal page – it shows all content – the nextpage tag is not being taking into account

See now: you deactivated SGR NP Plugin because I can see the nextpage code, but I still cannot reach non existing pages. Try loading a non existing page like https://talk2david.co.uk/2013/04/dasdsadas you can see that no 404 is loading, i see the same error:

    The page isn’t redirecting properly

Firefox has detected that the server is redirecting the request for this address in a way that will never complete.

Page loads perfectly on IE, Opera and Safari – I see a ‘No posts has been found’ message
