Not working on custom from please help

Hi

We use salesfroce ‘web to lead’ so all our forms are made in salesforce and then inserted manually on to our wordpress pages, this all works fine.

Id like to use this plugin but have not got the skills to sort out how to make it work.

Below is the form code. If you can help please let me know.

I have installed the plugin and activated it.
