Not working for Wocommerce Follow-up Email

Hi, I inserted the shortcode for Woocommerce Follow-up email(https://www.woothemes.com/products/follow-up-emails/) into the pop up. It appeared nicely but no action triggered when I pressed the submit button. FYI, the shortcode itself works on post, page and widget.
