Not working at all

  • The fields are displaying as they should but:
    – the checkboxes that are supposed to change the price (added according to instructions) are not working
    – the text entered in the text area is not visible anywhere.

    Tried all possible combinations in the settings. No js or php errors thrown.
    WP 5.2.2
    WooCommerce 3.6.5
    PHP 7.2.13
