Not Tracking As A Conversion – Google Analytics

I have followed your directions to a ‘t’! I’m able to see in real-time my form be sent, however, it’s not counting it as a conversion in Google Analytics. I can send you screenshots to be more thorough. Just seeing if you have immediate ideas.

I have this for the settings:
Goal Custom
Goal Type: Event
Goal Details: Equals to: form
Action Equals to: submission
Label Equals to:
Value: 1
