Not Sending Subscribers to Drip

I have setup a Drip feed on Gravity Forms to send subscribers to Drip. I have this setup on multiple other forms on my site and they are all working. This particular form though is not sending subscribers to Drip.

As a secondary issue, on the forms that are sending to my Drip account subscribers are not having the tag applied from the Drip feed.
