Not Receiving Emails or Showing in TRACKING

For some reason after battling with the positioning of the form on my site, (The cforms button puts a code that forces the form to the top of the page and I had to revert to the old code)

I am not not getting any emails and images to be uploaded to the form are not being uploaded to the server that they should be.

I’ve not ever had a problem making these forms work until now. And when I disabled the plugin to check something and then reactivated it all my original forms were gone. That has also never happened to me before.
