I just tried to submit the form (The “General Enquires” one) and while submission seems fine, I didn’t receive any e-mail too.
I understand that you already have e-mail notifications set in Form settings but since you are using SMTP can you please check “Advanced” tab of each of the notifications that you set and make sure that the “From Name” and “From Email” options are not set to name/e-mail different than ones configured in SMTP plugin?
Try either setting them exactly as in SMTP configuration or leaving them empty. Then check the same for “From Headers” section on “Forminator -> Settings -> General” page.
If it’s already set this way (either empty or same as SMTP configuration) or it doesn’t help, please export the form and share it with us so we could check and test it on our end.
To share exported form it’s best to put exported file on Google Drive, Dropbox or similar account and just post a direct link to it in response below.
