Not receiving comment reply mail notification

Hi,

Thank you for creating such a wonderful plugin.

I am using the plugin for 3 actions in my website.
1. Admin gets mail when user uploads new post (every post is private by default)
2. User (Post author) gets mail when admin comments on their post.
3. Admin is not receiving email when user/post author replies to admin comment.

Have no difficulty with 1 & 2. However I am not able to achieve the third step.
In 3rd case, sometimes the same user(post author) is receiving the mail about his comment rather than admin. (fires when new comment added is triggered in places of fires when comment is replied)
