Not feeding information

Hi,

I’m not receiving the information filled out on single text area, email, phone, address nor from the radio buttons.

I’ve changed the internal slug.
I’ve not attributed the initial value.
I’ve left ‘class name’ and ‘placeholder text’ blank
and
Have not enabled conditional logic.

I’m assuming it’s because of one of the above, which one should I fill in and what details do I need to put?
