Not changing to new default image.

I have (about almost a week ago) changed the image that I want to be my default image, however it hasn’t seemed to change on my site. Sometimes the new image appears but more often than not it is the old image.

The image should be a black image with my name in it, the older one was a white image with the logo cdma.
I have re-indexed the site numerous times, and have also attempted to fix the issue by deactivating and reactivating the plugin and this hasn’t helped.
