    Please go to your form back-end and remove the Contact us removing its mark. If you face any issue with it, please contact us to [email protected] and we will try to help you fix your issue.

    so simple to do those things – this is USER ERROR mate – not the plugin!

    I think you need to read the info and adjust your rating accordingly
