Not All Items of Form Displaying in the Post

Greetings,

I’ve recently purchased the pro version of this plugin. I am having issues with how my post appears after a completed form has been submitted. It appears that some of the fields that I have flagged to display on the front-end are not appearing on my post. Can you please advise?
