Not able to add media files

I can’t add media file to the post or set featured image
a blank page will pop-up with out loading the media data base in order to insert it in the post

also adding new media file will give the following error
Error fffff.jpg An error occurred in the upload. Please try again later.

But later you can find the fffff.jpg file in the library
