No wrapping and visible shortcode

Thanks for a nice plugin, just what I was looking for ??

I’ve found two issues so far:

1. When adding an image using the “Add Media” button in Text mode, the order of parameters in the img tag is different, e.g. the class parameter is written last instead of first. The result is that your plugin doesn’t wrap the img.

2. If several images are inserted right after each other with no line break, a visible [/caption] text can appear, and at least one of the images is not wrapped correctly.
