No titles showing for form fields

    Hi all, I thank you in advance for your help!
    Im fairly new so please excuse my poor use of correct terms

    Basically i built a site (in my sig/name) and i have add issues so i quickly rebuilt it and added contact form 7. I have no issues implementing it nor using it but when i have added it to my other sites, without issue i have managed to set it up how i liked with watermark text in box and a title above each box/field. Please see my site and id be most grateful if anyone can point out the issues with my form not showing the watermark in the box (text a customer would see before filling out box) and a title of the box above each. I even copied the form from my other site and it does the same on this site although works properly on my other site where it is also implemented. This makes me think its something external to the form.
    i have the form on my homepage and works fine at https://www.m-t-i.co.uk but is problematic at https://www.tvinstallationleeds.co.uk

    Its ok guys thanks anyway. i had a silly moment and forgot to manually input text above each box to act as a title! all the best ??
