No "Salesforce: API" setting menu item

We have installed the plugin and selected the API as our method, but no “Salesforce: API” item appears in the settings menu. Likewise, no “Salesforce” menu item appears in the left-hand Forms menu.
We did have to update our PHP version (from 5.2 to 5.4), if that makes any difference.
