No Image Icon in “Visual” Setting

I downloaded and activated the plugin. When I test it in a new topic or reply window, the image icon is not showing up in the “Visual” setting. In the “Text” setting it shows the “IMG”. What do I need to do to get the Image Icon to show in Visual mode?
