No failed password notification on failed log-in

It’s quite strange, since parameters are not filtered before loading headers, see below how it should work: https://www.icacreativeservices.com/login/?authentication=failed

Could you please try all again by disabling each plugin you are using? trying to find where is located the issue (more in depth)
