No button showing up on Visual editor

Hi there,
I can see the shortcode quicktag in HTML view, but there is no button in the visual editor.
I have ticked “Check to include a button to access this shortcode on the rich text editor.”
I am using WordPress 3.5.1 and v1.1.5 of this plugin with TwentyTwelve.
