    I had a similar issue after I installed the plugin. I deactivated all other plugins, thinking that its a plugin conflict, but the problem persisted.

    I then decided to flush my permalinks as a last resort. That fixed the problem.
    So for anyone else who have this problem. After adding your faq, go to permalinks and click the save button. Your faq will then appear properly on front end of website in accordian format.
