You need to use CSS to style it.

You can probably copy most of the styling from that site.

Learn to use the Chrome Developer Tools to help you see and test changes to your CSS.

Yep I did that, worked great. The only problem I am having is customizing the actual form to make the input boxes smaller, and adding the icons to the boxes.
