Newbie question on request/reply of data on WordPress page

Hi, I am new to WordPress/web programming and was wondering if I could solicit some feedback on the best approach to proceed with a website I was looking to create. The website would solicit users with a question and then they would then use a drop down box to select their answer. Once they have done that, they click a submit button where the selected answer is then used to query a database that matches and then displays another result set back to them in another field. Was wondering if there might be any existing plugin that might do this type of request/reply functionality or perhaps its something I would need to code myself.

Apologies if this is a simple query and would appreciate any advice.
