New Version Updates Mess Up Form

    The latest updates appear to add hidden form fields to the top of the form along with br tags that add breaks between the hidden fields. This creates an unusual amount of space between the intro text and the viewable form fields. Because they are br tags and not spacing done by css there is no way to close the spacing gap without modifying the plugin files, which breaks our ability to upgrade without hassle.

    Please comment and address this issue in the code.
