New terms not being added to A to Z list

  • My client was attempting to add new glossary items, she she added the term description and the group then published with no errors but the term was not added to the A to Z list.

    The last term was added in on 10/11/13 when I was building the site.
    I’m using the latest release of the Weaver Pro II theme on Word Press 4.0.1

    The address for the glossary page is https://getsmining.com/resources/mining-terminology-glossary/

    I tested on the a training site with the same configuration and have the same issue.

    This is how the short code is set up on the page
