New Hire Form (Page for Resumes and Video Interviews)

Hey,

I want people to be able to apply for Sales Representative positions through the website treenook.com. I’m hoping to create a form where people can attach a resume and submit a two to four minute video interview.

Are there any good predesigned forms that I could use? Right now I’m using gravity forms on the main page of the website, but it can’t accept resumes or videos.
