Need your help – TEXTAREA custom product details

Excuse me for my English, I’m russian, living in Italy..
So.. May you help me to make NON standard “product page”, it’s must be there textareas instead of product dates. I need a customer fill the form of some textareas, for ex:
ProductName Colour Size Price
ecc.. and then go to the checkout (without payment!)
So, same filds must be passed to the Orders.
Is it possible?? How many it may cost?
