Need Wp plugin for uploading chamber of commerce registration form

My woocommerce webshop has b2b sellings. A new customer must upload a recent copy of his chamber of commerce registration form. For this I search a plugin that does this when register. When register there must be 2 extra fields: reseller yes/no + upload document. In admin within in users listing both fields + option to read the uploaded document. The document must be related to registered user. Does someone of you know a plugin that fits my needs as described above? Would be nice to know.
