Need to add questionnaire form above products order form

On the homepage of my Woocommerce store I need to add a questionnaire form.

Right now on the homepage I display a table of products with cart button next to each product. People click on “add to cart” next to each item, and items get added to the cart. However, before people start adding items to cart I want to set a questionnaire form (couple of textarea fields) above the products table.

Right now I have the questionnaire placed at Billing and Shipping checkout page but because of the nature of the questions, and to avoid surprises for people, this form needs to be before people start ordering, and somehow this form needs to be tied with the overall order once it is submitted.

Any ideas how this can be accomplished?
