Need help locating a plugin for a decision flow

I have am having difficulties locating a plugin that will assist me in the following process.

For lack of better terms I want to set up a conditional flow process whereby the user is faced with a radio button choice and a submit button (there will be images and text on this and all subsequent pages as well).

Depending on which choice they make, when they hit submit they are taken to one page or the other. This next page also contains a radio choice and submit button (along with imgs and txt). Again a choice is made and submit is clicked.

The process can go on from one level to the next to the next etc.

I have searched and searched but I just haven’t found it probably because I’m using the wrong terms to search by.

Anyone?
