Need Assistance With Finding Optin HTML Code

I am using a wordpress template intrepidity theme that already has an optin form created on the home page. In my Theme Settings it is asking me to put in the GetResponse optin html code. I cannot find this anywhere. I do not want to use GetResponse’s form, I just want the optin html code. Hope someone can assist me. Thanks much.
